Certainly a full list Karen. You covered many of the most harmful ones. #12 in particular could be a catalyst as to why the others happen; trying to do too much means the advisor doesn't know exactly what they must be doing. If that is unclear, they'll struggle. Just wrote a book, The Distraction-Proof Advisor, that helps advisors with these issues.
